Emergency Response Information Kit (ERIK)

An Emergency Response Information Kit (E.R.I.K) can assist First Responders in situations where a patient may not be able to communicate their conditions, medical history or allergies. It provides essential information so responders can provide the correct care in an emergency medical situation.

The kit is ideal for seniors, chronically ill persons, those who live alone or have caregivers, and those with living with speech or language difficulties. Once completed, the kit should be placed on your fridge door, where emergency crews have been trained to look for it, ensuring it is readily available if needed.

You can receive your free kit at any Winnipeg Fire Paramedic Service Station or various Seniors' organizations
The WFPS Public Education Branch offers presentations to community groups to explain the ERIK program and the importance of completing a kit. To book a presentation, contact the WFPS by email fps-publiceducation@winnipeg.ca or phone 204-986-6380.

What does it contain

Your E.R.I.K should contain a completed health information form, which includes your name, address, health card numbers, next of kin, family physician, medical history, allergies, and medications. Your health care professional, pharmacist or family may assist you in filling it out. When your medical needs change, please print and complete a new form.

An E.R.I.K folder also contains:

Space for your health care directive (not mandatory to complete, but available if you choose)
Information about how to register intent to be a donor using Manitoba’s online organ and tissue donor registry.
A sticker for the front entrance of your house which alerts paramedics to look for your E.R.I.K.
A magnet to hold the information packet in place on your refrigerator door
