Access to fire reports
To obtain fire reports from the Winnipeg Fire Paramedic Service (WFPS), submit a request under the Freedom of Information and Protection of Privacy Act (FIPPA) on the Access to Information webpage.
The following types of fire reports are available from the WFPS through the FIPPA process:
- Apparatus Reports are summaries of firefighting actions and observations of the Fire Officer assigned to each responding apparatus.
- Fire Investigation Reports are detailed technical reports on fire cause and origin created by the WFPS Fire Investigation Unit for incidents they investigate. These reports are not available until several weeks after an incident due to their technical nature and are only created if there is an investigation.
- Incident Reports are summaries of property type, fire origin, spread, and, sometimes, cause if there is no investigation.
When submitting your FIPPA request, please specify which report you are requesting: Apparatus Report, Incident Report, Fire Investigation Report or all three.
For insurance companies requesting fire reports for the purpose of processing a claim, consent of the property owner or tenant is required. In cases where a neighbouring property is affected, consent of the neighbouring property owner or tenant is required. Please submit the Consent form after you submit your FIPPA request. Without written consent, FIPPA may require us to refuse portions of your request.

All concerns will be investigated thoroughly and promptly. In order for the WFPS to conduct an investigation details must be submitted in writing to avoid misinterpretation of the events while collecting the information. Please be sure to include the incident date and location as this information is required for WFPS to identify the incident. An acknowledgement letter will be sent to you within five (5) business days of receipt. Upon completion of the investigation a letter outlining the findings of the investigation and final disposition will be sent to you.

Access to Patient Care Reports
The patient's signature is required on the request form.
Please provide a copy of your government-issued photo I.D. (i.e. driver's license), and Manitoba Health card to FPS-Inquiries@winnipeg.ca, as we are required to establish proof of identity in order to release personal health information.
Requesting another person's PCR
To obtain another person's PCR you must indicate, on the Personal Health Information Access Request form, how you are authorized to act on the patient's behalf in accordance with section 60 of PHIA.
Documents for proof of identification/authorization include, but are not limited to: government-issued photo I.D. (i.e. driver's license), provincial health card, death certificate, power of attorney, executor of estate, etc.
Without written authorization from the patient, or proof of identification/authorization, PHIA may require us to refuse your request.
